Otoscopy Findings

Learning to perform otoscopy is not easy because the external auditory canal is obstructed by cerumen, otorrhea can be present, and the tympanic membrane is sometimes only partially visible. In children, it is even more difficult because of the narrow ear canal, the alignment of the tympanic membrane, and the restlessness of the child. 

Uncertainty in diagnosis can lead to overtreatment, as in the case of acute otitis media and unnecessary prescription of antibiotics. On the other hand, delay in diagnosis may lead to severe complications such as mastoiditis, meningitis and brain abscess in case of overlooked middle ear cholesteatoma.

A Social Dreaming Experience at the Time of COVID 19

The book describes the experience of four Social Dreaming Matrices held online between March and May 2020, during the first lockdown caused by the Covid 19 emergency. The pandemic isolated us and imposed prolonged contact with ourselves and our solitary thoughts. Against this backdrop, there was hope for change, a desire for a different kind of sociability and different forms of intimacy. On the basis of this evidence, our research supports the shift "from experiencing trauma to reacting to trauma", looking at a collective traumatic experience not only as something to be overcome but as an opportunity for a transformation that changes our mental schemes in relation to the external context. We have identified Social Dreaming as a privileged technique to overcome a collective traumatic experience, supporting its elaboration through collective feelings, new connections between intuition and rational thought, the discovery of community meanings. The authors's thesis is that the much-needed transition from 'magical thinking' to 'transformative thinking' takes place in a setting that is able to contain the anxieties of life's transitional phases, supporting the creation of new rituals and new   social bonds and sustaining the passage from “me” to a “wider we”.

Noninvasive Ventilation Outside Intensive Care Unit

This book aims to highlight the importance of the development of health conditions and demand for the application of noninvasive mechanical ventilation (NIMV) outside the intensive care units (ICUs); the diversification of possible scenarios outside the ICUs; the need to establish references that consolidate this phenomenon and the healthcare organizations models.

In the last decades the extension of the use of NIMV outside of the ICUs has led to the generation of protocols and to the creation of new in-hospital care models. In this field, the main determining factors are a better knowledge of technique, technological advancement, better monitoring capacity, the creation of multidisciplinary teams adequately trained in their application, and social and health events that have overloaded ICUs. All these elements have promoted the creation of these NIMV units outside ICUs. This new reality entails the need for clarification of concepts, recommendations, and analysis of how to planNIMV.

Although the literature that clearly determine the indications and aids on the use of NIMV is considerable, this volume, pointing out the diversity of different healthcare models to define how to organize NIMV outside the ICUs, shed a light and bring a clear benefit to the scientific community involved.

The book is structured in eleven main sections analyzing the epidemiology and trends for NIMV healthcare models and determining factors for these models outside ICUs.

The originality of the work, its clear clinical-practical impact and the multidisciplinary approach given by all healthcare professionals involved (intensivists, pneumologists, internal medicine and emergency medicine specialists, geriatricians, chest respiratory therapists, etc.) is very relevant for the thoroughness of the book.

At present, there is minimal and misleading knowledge of gastrointestinal dysfunction in Parkinson's disease among primary neurologists and gastroenterologists. This book reviews our current understanding of the GI involvement in PD, as many experiments suggest that the gut might be the site where the disease starts. Also discussed is how the gut dysfunction causes symptoms from the top (mouth) to the bottom (anal canal), and because of their frequency and severity, they contribute to disease progression and disability. In addition, the book covers the negative effects of small intestinal bacterial overgrowth (SIBO) and of a frequently silent stomach infection by Helicobacter pylori infection on the absorption of antiparkinsonian drugs, in turn causing motor fluctuations and inadequate treatment response.
