Informal Caregivers: From Hidden Heroes to Integral Part of Care

This book builds on the current trends in informal caregivers’ role in the supportive care of cancer patients (as well as other diseases) across the care continuum covering topics from the healthcare professionals and the users’ perspectives. Informal caregivers are a critical resource to their care recipients and an essential component of the health care system. The book introduces a comprehensive view of the topic and acknowledges the importance and the complexity of caregiving. Here lays one of the uniqueness of this book, which highlights the areas and the ways that for example interventions in specific settings/groups of patients can actually facilitate the caregiving process.
The increasing number of care-dependent people, the adoption of the principle “outpatient before inpatient”, the shift of care from inpatient to outpatient and the preference for home care (i.e. majority) are only some of the reasons that contributed to Informal caregivingbecoming a central feature of the health care landscape and will become even more prominent in the decades ahead.
The book draws on the experts’ high-end, current systematic research evidence and real-life examples on these topics to provide an insightful perspective on undertaking research within this context, and to demonstrate informal caregivers’ impact on patients’ outcomes. The structure of the book provides multiple perspectives to the topic and makes it appealing to a wide range of recipients including the nursing community, clinicians, social workers, researchers, policy makers, technology experts as well as postgraduate students especially to those practicing specifically in supportive care in cancer.
The book fills a gap in this field of expertise not only by familiarizing the reader with a wide range of topics to be considered but it also emphasizes on what the developments in the field in the future would needto take into consideration. Finally, current and future studies can be informed from the practices of preceding studies that are incorporated in the book.

Frontofacial Monobloc Advancement with Internal Distraction

Based on the experience at one of the largest centers for craniosynostosis and craniofacial malformations, this book reports on the clinical use of a rare technique known as Frontofacial Monobloc Advancement (FFMBA), one of the most rewarding procedures for upper- and mid-face reconstruction in patients with faciocraniosynostosis, a rare craniofacial anomaly associated with premature fusion of multiple cranial sutures presenting with brachycephaly, orbital proptosis, maxillary hypoplasia and anterior crossbite.

Technical refinements and strategic planning have made it almost a routine procedure at the French Reference Center for Craniosynostosis and Craniofacial Malformations.

This easy-to-consult, well illustrated book includes technical tips and indications for its use in Crouzon, Pfeiffer and Apert syndromes. It also provides a protocol, including a proposed management algorithm for faciocraniosynostosis. As such, it will appeal to craniofacial surgeons, especially those at large craniofacial centers.

Management of Dysregulated Immune Response in the Critically Ill

This book, part of the European Society of Intensive Care Medicine (ESICM) textbook series, deals with dysregulated host response a relatively new term introduced by the Sepsis-3 definitions in 2016. In contrast to systemic inflammatory response syndrome (SIRS) that has been used for nearly 30 years, this is a fundamentally new concept, which requires some sort of a paradigm shift in the way of thinking of critical care physicians. This change obviously requires time and after 5 years, time is ripe to summarize the knowledge in the form of a comprehensive book.

The book is split in 4 sections. In the first one, the host immune response is explained in 11 chapters, followed by the next part, summarizing the possible tools for assessment at the bedside. The third and the fourth parts focus on the effects of dysregulated immune response on vital organ function and on the possible ways of immunomodulation. Written by internationally acclaimed experts of the field, the bookis of value for all those intensivists and allied professionals working in ICUs.

Matrix Head and Neck Reconstruction

Head and neck reconstruction can be challenging due to the anatomical complexity of the area, its impact upon function, and the common need for adjuvant treatment postoperatively in cases of malignancy. Furthermore, the results are very visible and may significantly affect the patient’s ability to reintegrate socially. This unique surgical atlas is organized as a matrix of options, from simple to complex, covering all described anatomical areas of the head and neck. The organization of defects and reconstructive options, combined with illustrations and high-resolution intraoperative photography, make this the premier reconstructive reference that will be accessible when difficult reconstructive situations arise.
The matrix concept is a unique approach to organizing and presenting facial, head and neck defects. Most clinical cases present as a defect or as the potential for a defect after a resection. The matrix approach is a human cognitive approachto problem solving. This atlas allows the reader to search out their potential defects and study the possible reconstructive options available to them. This spectrum of possibilities may help the surgeon to decide whether to manage on the case on their own or refer it for more expertise. It is also a visible tool to assist surgeons in reviewing options and making decisions with patients in the informed consent process.
The matrix presents defects with internal consistency throughout the text and is organized in an anatomic order. Each anatomic sub-site is further subdivided into recognized clinical sub-units, and each potential defect is then illustrated in a standardized fashion to promote clarity and uniformity for easy reference. Commonly associated defects are linked. Each specific defect is then addressed, presenting reconstructive options of increasing complexity, with photography to show a staged approach to pre-reconstruction, site preparation, flap selection, flap inset, and final results acutely post-operatively and at a delayed time point.
Matrix Head and Neck Reconstruction combines and distills the work of major head and neck reconstruction and facial plastic surgery to provide a broad and deep resource that is highly pictorial and practical in providing a scalable approach to reconstruction.

Lung Cancer Screening

This book is a comprehensive guide to lung cancer screening for clinicians, healthcare systems, community leaders, and public health officials with the hope of creating a more equitable landscape in both lung cancer screening and lung cancer-related outcomes, at local, state, and national levels. Authors take a new approach to primary and secondary lung cancer prevention that is in the early stages of adoption in the United States. The last decade ushered in recognition of screening as an effective intervention, but unfortunately, despite the wide acceptance of the importance of this new screening modality, nationally, not more than 5% of eligible subjects have undergone screening to date in the United States, although in some states uptake has reached as high as 16%. As is common with any new preventive cancer screening, racial and socioeconomic disparities emerge in utilization, stage at diagnosis, and mortality. Over time, these disparities decline, but consequential differences endure. Therefore, it is critical to establish equitable screening practices.

The true measure of the effectiveness of any lung cancer screening program needs to be viewed through the lens of its impact on populations, including those most affected by the morbidity and mortality of smoking-related illness and lung cancer. As such, this book emphasizes a number of important public health topics, including community outreach to vulnerable populations, social justice issues, addressing stigma and fatalism in the general community, and the use of geocoding to assess a program’s impact at a population level. This book weaves traditional topics related to lung cancer screening, such as promoting initial and repeat screening, interpreting Lung RADs, and managing the follow-up of findings, into the population perspective in order to present a unified, comprehensive approach to the subject. Further, it serves as a guide that health systems, health care professionals, community leaders, and other stakeholders can use to achieve the promise of lung cancer screening.

Controversies in Thyroid Nodules and Differentiated Thyroid Cancer

This book provides a practical, evidence-based approach to controversies in the management of thyroid nodules and differentiated thyroid cancer (DTC). Thyroid nodules are a commonly encountered clinical entity, identified in 2-6% of the population by palpation, and 20-35% by ultrasound. DTC is the solid organ cancer with the most rapidly increasing incidence (>50,000 new cases in the U.S. annually). Despite extensive research and multiple published comprehensive practice guidelines, numerous controversies remain within this field covering a wide range of issues, including incidence and epidemiology, radiographic evaluation, criteria for needle biopsy, cytopathologic and molecular diagnostics, active surveillance vs. operation for low-risk tumors, extent of surgery, indications for radioactive iodine treatment and external beam radiation, and optimal protocols for follow-up.

The book takes a case-based approach: each chapter will begin with a patient presentationhighlighting a particular controversial subject in thyroid nodules and DTC. The remainder of the chapter discusses the controversy within the context of current scientific understanding and clinical practice, using the latest evidence-based practice guidelines. Chapters conclude with a final review and discussion of the opening case, with options and recommendations for next steps provided. The recommendations are evidence-based and practical, intended to help guide practitioners in their ongoing care of patients with thyroid nodules and DTC. Each chapter was written by a recognized expert in the area of controversy, including endocrine surgeons, endocrinologists, radiologists, medical oncologists, and nuclear medicine specialists.

Controversies in Thyroid Nodules and Differentiated Thyroid Cancer: A Case-Based Approach is an excellent guide for oncologists, endocrinologists, otorhinolaryngologists, surgeons, and practitioners of all types that care for patients with thyroid nodules and DTC.

Fetal Cardiology

This thoroughly updated practical book describes a systematic approach to ultrasound examination of the fetal heart based on accepted screening recommendations.  The content is enhanced by images and videos of both normal and abnormal sonographic findings that brings the topic to life and provides the reader with critical clinical information on best practice.

Fetal Cardiology: A Practical Approach to Diagnosis and Management goes further than simply describing core screening views. It includes extended views of the fetal heart, the use of Doppler techniques and assessment of fetal cardiac function. “Variants” that can be encountered in practice are described as well as the features of the major groups of cardiac abnormalities and fetal arrhythmias. Because the authors include experienced fetal and paediatric cardiologists, the focus is not only on diagnostic features but also the approach to postnatal care and prognosis. This content is enhanced by inclusion of chapters relating to associated fetal abnormalities, the genetics of congenital heart disease and new imaging modalities such as MRI of the fetal heart.

This book equips those using ultrasound to image the fetus with a clear concise reference to meet the challenge of new guidelines and to expand their knowledge of complementary echocardiographic techniques and management. It details why prenatal recognition of congenital heart disease is being prioritised to allow for parental choice, recognition of associated abnormalities and improvement of postnatal outcome. As such, this book is a vital resource for all professionals, whether they be a cardiologist, fetal medicine specialist, sonographer or midwife.

The Patient-Centered Approach to Claudication

In a publication in the Lancet by the Global Burden of Disease Collaborators, peripheral artery disease (PAD) was noted to be a major public health concern due to its “high and rising prevalence.” Intermittent claudication (IC) is the most common symptomatic manifestation of PAD, causing significant disability and reductions in health-related quality of life. While, at the global level, there is concern for gross under-treatment of PAD, here in the US, there has been growing concern for over-treatment of IC. It is clear that IC is a topic that warrants a dedicated, thorough, evidence-based resource that can help vascular specialists gain a comprehensive understanding of IC and how to best approach and engage patients in their care.

This book reviews the current literature and clinical practices related to a patient-centered approach to claudication, from the anatomy, physiology and epidemiology related to IC to diagnosis, risk factors and management techniques, including pharmacology, exercise therapy, and open and endovascular revascularization. There are sections dedicated to outcome disparities, the psychological impact of IC, patient preference science and cost effectiveness. There is also a patient-facing brochure and educational poster that can be accessed and downloaded freely. The content is geared toward a range of providers, from practicing vascular specialists and primary care providers to those in vascular training programs. Future directions for treatment and research, as well as an assessment of unmet needs, round out the presentation.

Street Mental Health Handbook

This book grounds the work of street mental health within the lived realities of the people it serves, emphasizing the importance of trauma and context of care, and how these factors shape the experience of homelessness and engagement with mental health care. With the rise in homelessness has come a growing movement of street medicine—healthcare delivered directly to individuals who live on the streets, in encampments, or within emergency shelters. While physical health services are relatively well-integrated into street medicine models, mental health care remains underdeveloped and poorly resourced. There is a pressing need for guidance, reflection, and solidarity in this space.

The authors have nearly fifty years of combined experience working in street-based mental health services across North America. Much in this field has not yet been formally researched or codified; many insights have been gained through trial, error, and the wisdom of those served. The authors’ goal is to share what they have learned in a way that is honest, grounded, and helpful. The opening chapters discuss the individual, collective, historical, and structural traumas that are common in the vulnerably housed population and how these should inform engagement, diagnosis, and treatment. Subsequent chapters explore the complexities of and practical strategies for conducting mental health assessments and prescribing medications on the street. The authors discuss the importance of building and working in teams to adapt to the chaos that street medicine clinicians and their patients frequently encounter. The book closes with a discussion of psychosocial interventions for individuals experiencing homelessness, emphasizing their potential effectiveness despite implementation challenges. Chapters emphasize harm reduction and include case studies and dialogues to aid understanding.

The primary audience of Street Mental Health Handbook is front-line clinicians—physicians, nurses, outreach workers, social workers, and others—who are new to the field of street mental health or are seeking to deepen their understanding of it. For those already embedded in this work, the authors hope it will serve as a practical and reflective guide that acknowledges the unique challenges and rewards of working with people who are homeless and living with mental illness.
